Parent Agreement & Liability Waiver.

I understand that swimming and water activities involve inherent risks, including but not limited to injury, illness, or drowning. I voluntarily choose to have my child participate in lessons with JerryBuilt Swim LLC and accept all associated risks.

I acknowledge that JerryBuilt Swim LLC and its instructors are not responsible for accidents or injuries that may occur during or as a result of participation.

I understand that lifeguards may not be present at all lesson locations, especially at private or backyard pools.

I acknowledge that the instructor is not acting as a lifeguard and is not responsible for supervising swimmers outside of direct instruction.

I accept full responsibility for my child’s safety outside of active instruction.

For swimmers under the age of 2, a parent or guardian is required to be in the water and actively participate.

For all other swimmers, I understand that I am responsible for supervision before, during, and after lessons, even if I am not physically in the water.

I agree to inform JerryBuilt Swim of any medical conditions, physical limitations, or concerns that may affect my child’s safety or participation.

I confirm that my child is physically able to participate in swim instruction.

In the event of an emergency, I authorize JerryBuilt Swim and its instructors to seek medical assistance on behalf of my child, including calling emergency services if necessary.

• All lessons must be paid in full prior to the scheduled session.
• Packages (4-pack, 8-pack, etc.) must be paid upfront before use.

• A minimum of 24 hours notice is required to cancel or reschedule a lesson.
• Cancellations made with less than 24 hours notice will result in the loss of that lesson.
• JerryBuilt Swim may, at its discretion, offer occasional makeups, but they are not guaranteed.
• If JerryBuilt Swim cancels a lesson, a full makeup or credit will always be provided.

I understand that in some cases, another qualified JerryBuilt Swim instructor may conduct the lesson instead of Jerry McMonagle. All instructors operate under the same standards, structure, and expectations.

JerryBuilt Swim reserves the right to:

• End a lesson early if a situation is unsafe
• Remove a swimmer from the lesson if necessary
• Refuse or discontinue service at any time

No refunds will be issued for lessons ended early due to unsafe or disruptive behavior.

I understand that lesson structure and conditions may vary depending on the location (backyard pool, community pool, or rented facility).

I agree to follow all pool rules and understand that access, entry fees, and conditions may differ based on location.
